This person prepares memos, letters, summaries and reports maintains ongoing documentation of actions taken and ongoing contacts that are affiliated with each consumer and assists with the completion of various entitlement forms. The individual prepares case histories, social assessment forms and care plans with obtainable goals and the action taken to aid the consumer in attaining the goals submits and maintains required information in the PA Department of Aging standardized computerized system, SAMS. The caseload will include bi-lingual consumers and translation assistance will be provided to the Agency and its employees as requested and directed.
